Please read the directions carefully and complete both forms listed below: Form A is to be completed regarding the student information; Form B is to be completed regarding parent/guardian information.

Form A is for the student and/or registered children attending the Diamonds and Pearls Enrichment Program.
All attendees must complete the registration form before attending any session(s). Siblings or attendees within the same household must complete a registration form (Form A) for each child attending the program. 
Form B is for the parent and/or guardian to complete. Please list the name of your child (or children) in the designated area with the form.
